Online lab service request submissions and digital Chain of Custody form in Relay is currently only supported for samples submitted to Luminultra's Baltimore laboratory.
If you are submitting lab samples to our Baltimore Lab (for Energy, Water and Industrial lab services), our online testing platform Relay™ now includes a web solution for submitting lab-testing requests via a digital Chain of Custody form. This new feature replaces the need to complete and submit a paper Chain of Custody form with your shipment.
Use the digital form to provide your customer information, sample details, requested analyses, payment method, and shipping information in one place. After you submit the form, Relay will provide a Chain of Custody number and packing slip. Print the packing slip and include it in your sample shipment so our Lab Services team can match your physical samples to your request.
Testing and turnaround time begin once the physical samples are received by Luminultra.
Before you begin
Before starting a new lab service request, make sure you have:
- Your contact and company information
- Your sample names, sample types, amounts, and collection dates
- The analyses you want performed
- Your payment method, either credit card or purchase order
- Any special handling instructions for the samples
- The email addresses of any collaborators who should receive results
You do not need to enter credit card details in Relay. If you select credit card as your payment method, this only indicates that you plan to pay by card when you are invoiced after results are ready.
Start a new lab service request
- Sign in to Relay.
- Open the menu in the top right corner.
- Select Lab Service Requests.
- Select New Lab Request.
- Complete the Testing Request / Chain of Custody form.
You can also return to Lab Service Requests later to view previous requests, review request details, or reprint a packing slip.
Complete customer information
In the Customer Information section, enter the contact and company information for the request.
Required fields include:
- Contact name
- Company
- Street address
- City
- State or province
- Postal or zip code
- Country
- Contact phone
- Contact email
You can also enter a quote number if applicable.
Please make sure this information is accurate and consistent. It helps Luminultra process the request, match samples to the correct customer, and support reporting.
Complete payment information
In the Payment Information section, select one of the available payment options:
- Credit Card
- Purchase Order
If you select Credit Card, you are indicating that you plan to pay by card when you are invoiced. Credit card details are not collected in Relay.
If you select Purchase Order, enter the purchase order number in the Purchase Order # field.
Add collaborators (optional)
In the Collaboration section, enter the email addresses of any collaborators who should be authorized to receive results.
To protect confidentiality, results will only be sent to the email address used to submit the form and the collaborators listed on the request.
Enter sample information
In the Sample Information section, enter the details for each sample you are submitting.
For each sample, provide the required information, including:
- Sample name
- Sample type
- Amount
- Collection date
You can also add a Sample Site and Sample Location. These fields are encouraged because they provide useful context for reporting, but they are not required.
The Sample Site and Sample Location fields will show any ATP sites already available in your Relay account. You can also add new sample sites or locations while completing the Chain of Custody form. New sites or locations added in this form are only used for the lab service request. They will not be added to the ATP testing side of Relay.
Each form can include up to 10 samples. If you need to submit more than 10 samples, complete and submit one form, then start another form for the remaining samples.
Clearly write the sample number shown in Relay, such as 1, 2, or 3, on each individual sample container before shipping. This helps the lab match each physical sample to the correct row on your request.
You can also enter any special handling requests in the Special handling requests field.
Request a rush order
Use the Rush order? option if you need rush processing.
A 50% surcharge will be applied to the sample cost for rush orders. Rush orders are not available for sequencing analyses.
Select analyses required
In the Analyses Required section, select the analyses you want Luminultra to perform.
You can use one of the presets to quickly select common groups of analyses:
- All
- Industrial
- MST
- Pathogen
You can also select individual analyses from the available categories.
Important: the analyses selected on the form will be performed on all samples submitted with that request. If different samples require different analyses, submit separate forms.
Review and submit
When the form is complete, select Review.
Review all information carefully, including:
- Contact information
- Payment information
- Collaborators
- Rush order selection
- Sample details
- Requested analyses
- Special handling requests
If changes are needed, select Edit. When everything is correct, select Submit.
After submission, Relay will display your Chain of Custody number and packing slip. Relay does not send an email confirmation, so please print or save the packing slip from the confirmation screen.
Print and include the packing slip
After submitting the form, select Print to print the packing slip.
Place the printed packing slip inside your sample shipment. The packing slip helps Luminultra match your physical samples to the digital Chain of Custody request.
If you forget to include the packing slip, Luminultra should still be able to identify your samples using the shipping information. Including the slip helps make processing faster and easier.

Before shipping, make sure:
- Each sample container is clearly labelled with the sample number from Relay.
- The packing slip is printed and included in the package.
- Samples are packaged securely for shipment.
- Any special handling instructions are included on the digital request.
- You have followed any shipping, preservation, temperature, or customs requirements that apply to your sample type or requested analyses.

View or reprint a request
To view a submitted request:
- Sign in to Relay.
- Open the menu in the top right corner.
- Select Lab Service Requests.
- Find the request in the View Requests table.
- Select Details to review the Chain of Custody information or select Packing Slip to reprint the packing slip.
Make a change after submitting
If you notice a mistake after submitting a request, contact help@luminultra.com and explain what needs to be adjusted.
Luminultra will update the request for you. Once updated, the corrected information will be reflected in your Relay account.
